Who should use this checklist & when?
While doctors and nurses primarily use this checklist, any caregiver who notices changes in someone with dementia can discuss these points with medical staff.
Quick Assessment Guide
Check Physical Health
- Look for signs of infection, pain, or discomfort
- Check if the person is constipated or dehydrated
- Ensure they can see and hear properly
- Make sure they're eating and drinking enough
Review Medical Factors
- Contact their doctor for a check-up (blood tests may be needed)
- Review any recent medication changes
- Note any recent surgeries or medical procedures
- Check if they're wearing their glasses and hearing aids
Monitor Behaviour Changes
- Watch for acute (sudden) confusion or disorientation
- Note any unusual mood swings
- Look for signs of hallucinations or delusions
- Track recent sleep patterns
Create a Supportive Environment
- Reduce noise and activity around them
- Keep lighting gentle and not too bright
- Help orient them to time and place
- Encourage family visits for comfort and familiarity
If Delirium is Confirmed
- Follow your long-term care setting’s Delirium Treatment Plan
- Treat the underlying cause
- Keep the environment calm and familiar
- Ensure proper use of glasses and hearing aids
- Welcome family visits to help with orientation
